I’m trying to keep every case plan truly individualized even on weeks when the calendar is packed. With a 62-person caseload, I’m doing 15-minute weekly check-ins and updating SMART goals in our OMS, but I worry that box-checking misses the small wins — like a quiet de-escalation or a peer-mentor shift — so how are you capturing those micro-milestones and making sure they show up in progress notes and release recommendations?
I started doing a 60-second ‘wins’ memo for each check-in in the OMS — one line like: 10/25 ‘quiet de-escalation on the bus’ — and tag it (#deescalation, #peermentor) so I can filter them into monthly summaries. If your system won’t do audio, I just paste the same one-liner into the SMART goal comment, but I cap it at 140 chars so it stays quick.
During the 15-minute check-in, I end with, “what’s one thing you did this week that no one saw?” and drop that exact one-liner into the SMART goal comment in the OMS so the micro-win sits with the goal it moved. It’s a 20–30 second add, and on packed days I just dictate it on mobile; if the notes get noisy, I prefix those lines with QQ: so I can filter them fast later.
I do a quick voice-to-text “two-word evidence” right after the check-in — context + verb — like: bus — stayed calm; shift — backed up peer; prefix with MW: so it’s searchable, and it leaves a breadcrumb trail without slowing me down. Building on @efrank_91, I also add a single emoji for type (
calm,
peer) to make weekly reviews pop. If your OMS fights this, I jot them in Apple Notes and paste the MW lines in a Friday batch.
But building on @g_hughes25, I grab a 5–7 word client quote at the end (“what would you call that win?”), prefix it with CQ: plus setting (e.g., bus), and later search CQ to roll those into reviews; if they won’t be quoted, I switch to a quick ABC line (setting–behavior–impact).
