Tracking Progress for Individualized Rehabilitative Plans

I’ve been thinking a lot about how we track inmate progress toward individualized goals. Recently, we started using a new software tool that helps us monitor their development more closely. It’s still early days, but it’s encouraging to see how tailored plans can positively impact behavior and readiness for reentry. Has anyone else had success with similar approaches?
